The PICASO project stands for: A Personalised Integrated Care Approach for Service Organisations and Care Models for Patients with Multi-Morbidity and Chronic Conditions.
It has received funding from the European Union’s Horizon 2020 research and innovation programme under grant agreement no 689209. The project started in February 2016 and will last until February 2019.
